Free Health Screenings and Personal Health Assessment

Free Health Screenings and Personal Health Assessment will be offered in 2008 by the Kansas Health Policy Authority (KHPA).  On site health screenings will be held at Pittsburg State University March 10-12, 2008, in the Crimson and Gold Room, Overman Student Center.  Other sites are available throughout the state at other dates.  Click here for a complete list of sites.  

Participants that complete the Personal Health Assessment will receive a $50 gift card.  Click here for a list of gift cards available.

What’s included in the Free Health Screening?

Eligible persons will be able to get a Free Health Screening that will include:

  • Blood Pressure
  • Height and Weight
  • Body Mass Index Calculation
  • Total Cholesterol (finger stick) included HDL & LDL
  • Triglycerides
  • Glucose

Summit Health, a nationwide health provider, will conduct the screenings. Appointments can be scheduled between 7:00 a.m. and 12:00 noon. There will be three (3) people scheduled per time slot. The screenings take approximately 15 minutes and results will be provided immediately.  The health screenings will require participants to fast, which means no food (only water or black coffee) for 9 hours prior to test.  Please be sure to take any medications per your regular schedule.

Who is eligible to participate to participate in the Free Health Screenings and Personal Health Assessment?

  • All employees enrolled in the State Health Plan
  • All benefits-eligible employees who waive State Health coverage
  • All spouse/dependents 18 years of age or older who are enrolled in the State Health Plan
  • Retirees and spouses/dependents age 18 or older who are enrolled in the State Health Plan

How do eligible participants schedule the Free Health Screenings?

To schedule your screening appointment, you must use the KHPA Dialog Center.   

Important information regarding the Online Health Screening Appointment Schedule Tool:

  • Register at the KHPA Dialog Center – https://www.thedialogcenter.com/healthquest/
  • In the First Time User section, click on “Go to Registration Page” link.
  • You will need to create a login/user name and password which must have at least 6 letters and 1 number.
    • The user name must match exactly how HRS has you in the personnel system (e. g., Debra Amershek, not Debbie Amershek)
  • You will need to click on the "Click here to make your screening appointment" link.
  • You will be asked to register your information again on the Summit Health web site.
    • First Time Users click on "Click Here to Register" (blue box.)
  • You must enter an email address in the registration process.
    • Each email address must be unique – more than one person cannot use the same email address.
    • If you do not have an email address, you can use “name@xxxxx.yyy”
    • The email address is only used to send an email notification of appointment and a reminder message.
    • If you do not have an email address, you should print screen the appointment confirmation page.
